The Dementia Navigation Program (DNP) is now available for caregivers of those who have dementia.
DNP offers caregivers a total of three visits over a six-month period with occasional phone calls in between visits when needed. Topics covered during the visits will be one-on-one coaching to cope with dementia related behaviors such as wandering, repeating (looping) of conversations, anger/aggression and other behaviors the caregiver may be encountering. The caregiver will also be assessed for caregiver stress, depression and burnout.
Eligibility for the Dementia Navigation Program
- Adults age 60 and older
- Participant and caregiver live in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo or Osceola counties
- Caregiver is willing to meet three times over a six month period
