The Dementia Navigation Program (DNP) is now available for caregivers of those who have dementia.
DNP offers caregivers a total of three visits over a six-month period with occasional phone calls in between visits when needed. Topics covered during the visits will be one-on-one coaching to cope with dementia related behaviors such as wandering, repeating (looping) of conversations, anger/aggression and other behaviors the caregiver may be encountering. The caregiver will also be assessed for caregiver stress, depression and burnout.
Eligibility for the Dementia Navigation Program
Adults age 60 and older
Participant and caregiver live in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo or Osceola counties
Caregiver is willing to meet three times over a six month period
