Programs intended to provide assistance to caregivers in understanding and coping with a broad range of issues associated with caregiving.  Examples are education programs on the physical, emotional and spiritual aspects of caregiving as well as current research and public policy concerns. Support activities for caregivers including support groups, counselling and connection with community resources. Also training programs pertaining to techniques for providing personal care services to care recipients.

Services needed to maintain the home in a clean, sanitary, and safe environment.  This service includes heavy household chores such as washing floors, windows and walls, tacking down loose rugs and tiles, moving heavy items of furniture in order to provide safe access and egress inside the home.  This service also includes yard maintenance (mowing, raking, and clearing hazardous debris such as fallen branches and trees) and snow plowing to provide safe access and egress outside of the home.

Community Living Supports facilitate an individual's independence and promote reasonable participation in the community. This may include meal preparation, laundry or assistance with activities of daily living (bathing, dressing, eating and personal hygiene).

Nutritious meals served in various locations. Lunch is often coordinated with a variety of activities such as exercise programs, cards, crafts, bingo, games, health screenings and nutrition education.

Professional level counseling services seek to improve the individual’s emotional and social well-being through the resolution of personal problems and/or change in an individual’s social situation. 

Durable medical equipment and supplies include devices, controls, or appliances that enable individuals to increase their abilities to perform activities of daily living, or to perceive, control, or communicate with the environment in which they live. 

Programs for the prevention and treatment of elder abuse, neglect, and exploitation. Presentations are made to aging subcontract organizations, professional and paraprofessional groups through community outreach and  public education.  Case consultation with other local agencies such as Adult Protective Services, Department of Human Services, law enforcement, health care professionals, and community mental health.

Support services in kinship care situations where an individual aged 60 or over is the primary caregiver for a child no more than 18 years old.  Kinship support services include respite care, education, support and training provided at locations other than the client’s residence.

Programs designed to be an effective tool in improving the health of older adults.  Programs offered may include:  A Matter of Balance, EnhanceFitness, Arthritis Foundation Exercise Program, and PATH. To find a schedule of classes, click here

Home delivered meals service is the provision of at least one nutritionally sound meal per day to persons who are dependent, aged, or physically disabled and unable to care for their nutritional needs.  The unit of service is one meal delivered to the client's home that provides a minimum of one-third of the current recommended dietary allowance for the age group as established by the Food and Nutritional Board of the National Research Counsel of the National Academy of Sciences. 

Physical adaptations to the home that are necessary to ensure the health and welfare of the participant or that enables them to function with greater independence in the home. Examples may include installation of ramps, grab-bars, widening of doorways, modification to bathroom facilities, etc.

Services provided to participants unable to care for themselves that are furnished on a short-term basis to provide relief to a caregiver. Services are provided in the individual's home.

Provision of legal advice, counseling, and representation by an attorney to low-income older adults.

Assists residents and prospective residents of long-term care facilities in understanding their rights and resolving complaints to improve care.

Individual and continuous care (in contrast to part time or intermittent care) provided by licensed nurses within the scope of State law.  These services are provided to an individual at home.

Services provided to participants unable to care for themselves that are furnished on a short-term basis to provide relief to a caregiver. Services are provided in a private place of residence other than the participant's own home.

An electronic device that enables participants to sercure help in an emergency. The participant may also wear a portable "help" button to allow for mobility. The system is connected to the phone and programmed to signal a response center ones a "help" button is activated. The response center is staffed by trained professionals.

Services offered in order to enable individuals to gain access to community services,  activities and resources.
