Adult Day Service  

Daytime care for functionally impaired elderly persons provided through a structured program of social and rehabilitative services.

Bathing and Personal Care services are sometimes provided-inquire at individual organizations.

Comprehensive assessment for medically frail clients. Care management identifies client needs and arranges for supportive services that maintain quality of life and independence, as much as possible.

Caregiver Support  

Programs intended to provide assistance to caregivers in understanding and coping with a broad range of issues associated with caregiving. Examples are education programs on the physical, emotional and spiritual aspects of caregiving as well as current research and public policy concerns. Support activities for caregivers including support groups, counselling and connection with community resources. Also training programs pertaining to techniques for providing personal care services to care recipients.

Congregate Meals  

Nutritious meals served in various locations. Lunch is often coordinated with a variety of activities such as exercise programs, cards, crafts, bingo, games, health screenings and nutrition education.

Elder Abuse Prevention Education  

Programs for the prevention and treatment of elder abuse, neglect, and exploitation. Presentations are made to aging subcontract organizations, professional and paraprofessional groups through community outreach and public education. Case consultation with other local agencies such as Adult Protective Services, Department of Human Services, law enforcement, health care professionals, and community mental health.

Grandparent Support Group  

Support services in kinship care situations where an individual aged 60 or over is the primary caregiver for a child no more than 18 years old. Kinship support services include respite care, education, support and training provided at locations other than the client’s residence.

Home Delivered Meals  

Home delivered meals service is the provision of at least one nutritionally sound meal per day to persons who are dependent, aged, or physically disabled and unable to care for their nutritional needs. The unit of service is one meal delivered to the client's home that provides a minimum of one-third of the current recommended dietary allowance for the age group as established by the Food and Nutritional Board of the National Research Counsel of the National Academy of Sciences.

Homemaking  

Performance of routine household tasks to maintain an adequate living environment for older individuals with functional limitations such as laundry, ironing, meal preparating, shopping for necessities (including groceries) and errand running, light housekeeping tasks (dusting, vacuuming, mopping floors, cleaning bathroom and kitchen, maintaining safe environment).

MI Choice Medicaid Waiver  

MI Choice is a Medicaid-funded program that provides in-home services and home care assistance to seniors and persons with disabilities who are at risk of being placed in a nursing facility. Program participants are partnered with a care management team, made up of a licensed social worker and a registered nurse, to develop a care plan centered on the individual’s needs.Click here for more information

Nursing Facility Transition  

The Nursing Facility Transition (NFT) program helps seniors currently living in nursing homes return to independent or semi-independent living within their community.
