AAAWM contracts with local businesses to provide the various funded services offered to older adults, adults living with disabilities and caregivers. Our Partners provide services based on quality standards established for each service area and incorporate a person-centered model of service delivery. 

Kent County Senior Millage Partners

Kent County Senior Millage Partners receive funding from the Millage to provide services for older adults age 60 and over in Kent County. Every year there is an open proposal process that provides any incorporated entity the opportunity to submit a Letter of Intent to request funding for a service for older adults in Kent County.

Older Americans Act (OAA) Partners

Every three years an open proposal process provides any incorporated entity the opportunity to submit a proposal to provide one or more of the defined fundable services.  If an agency is selected, their contract will remain in effect for 3-years though funding can vary from year to year.

Purchase of Service Partners

AAAWM has established Purchase of Service (PoS) Agreements with a large pool of service partners across the nine county region to serve clients under the MI Choice Medicaid Waiver, Care Management and Veterans Health Administration programs. Some of the most frequently purchased services include:  Homemaking, Personal Care, In-Home Respite, Home Delivered Meals, Personal Emergency Response Systems, and Adult Day Care.

Potential new PoS Partners may apply to become a part of the provider pool. Purchase of Service Agreements may also be established throughout the fiscal year in order to fill a specific area of need.
