The MI Choice Waiver Program is funded by Medicaid and is designed to provide a variety of in-home services designed to meet an individuals long term care needs in a community based setting.  To qualify, individuals must be at least age 65 years of age (or aged 18 to 64 and certified disabled) be medically eligible for nursing home level of care, meet financial eligibility guidelines and require waiver services to continue living in the community.
