Connecting West Michigan Seniors with In-Home Care Services
Most seniors desire to stay in their own home as they age. The Area Agency on Aging of Western Michigan provides older adults and their families with resources for in-home care services and home care assistance programs in the Grand Rapids area.
Benefits of In-Home Care
Seniors who live in their own homes as they age generally experience a greater sense of well-being. They are able to maintain the sense of independence living on their own provides, and families have peace of mind that their loved ones are well cared for. Seniors living independently are able to maintain active social lives, frequently connecting with friends, family, and neighbors.
Seniors wishing to participate in our Care Management program receive an in-home assessment to determine if they qualify for in-home care services. Each client is matched with a care management team, made up of a licensed social worker and a registered nurse, to provide on-going case management. We arrange services through a pool of community organizations that adhere to the strictest guidelines for senior care and safety.
Choices for Independence
The Choices for Independence program helps adults age 60 and over continue living independently in their communities for as long as they choose. We help families connect with community programs that provide in-home care so their loved ones can age safely in their own homes.
MI Choice is a Medicaid-funded program that provides in-home services and home care assistance to seniors and persons with disabilities who are at risk of being placed in a nursing facility. Program participants are partnered with a care management team, made up of a licensed social worker and a registered nurse, to develop a care plan centered on the individual’s needs.
Nursing Facility Transition
Our Nursing Facility Transition program helps seniors currently living in nursing homes return to independent or semi-independent living within their community. Transitioning back into independent living can be difficult for both the senior and his or her loved ones, but the NFT team helps individuals and families during the entire transition.
