Community Living Program:

Choose to stay in your own home with the support you need.

Choices for Independence (Community Living Program) educates older adults who do not qualify financially for assistance in ways they can stay in their own home longer. Individuals and families are encouraged to pair current resources with community programs to prevent nursing home placement. Options counseling is provided by a licensed Social Worker.

 

Qualifications:

  • Live in Allegan, Ionia, Kent, Monctalm or Newaygo county
  • Desire to stay in your own home with personal and community resources
  • Have a hard time taking care of your need without help
