Choices for Independence:

Choices for Independence helps individuals age 60 and over continue living independently in the community for as long as they choose. Individuals and families are encouraged to pair personal resources with community programs to optimize safety and needed care. A consultation is provided by a licensed Social Worker.

 

Qualifications:

  • Age 60 or older

  • Live in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo and Osceola Counties

  • Desire to stay in your own home with personal and community resources

  • Have a difficult time taking care of your needs without assistance
